This form helps us gather the information required to prepare your pet’s health certificate for travel. Please complete it prior to your appointment, especially if your pet is traveling by air or crossing state/international borders.

Health Certificates cannot be done younger than 8 weeks of age.
This is different than a normal health certificate). We do not do any international certificates. Owner/Seller will need to call Air Animal or find a veterinarian that can/will do international certificates.
If with Purchaser or Transporter, need the following information: See below
It is the owner’s/seller’s responsibility to know what is needed for the airline to allow pet/s to travel onboard. Most travel certificates need to be done within 10 days of the flight. If seller doesn’t have the above information at the time of making the appointment, please make sure to have it when arriving at the clinic.
Please note that any animal who has any positive test result CANNOT receive a certificate until results are negative. Additional costs may occur for recheck appointments/tests.
